Video Loan
Agreement

Carefully read the following terms and
conditions before executing this agreement.
To
be eligible to borrow videos, a signed Video Loan Agreement must be on
file at the Lawton Area Prevention Resource Center. This Agreement must
be updated annually.
Videos
are loaned for a period of one week at no charge. No more than
two videos will be loaned to an individual at any given time. An
extension of an additional week may be requested by phone and granted
providing there is no one on a waiting list for the video. Consistent
late return of videos will result in denial of future video loan
privileges.
If a
video is lost or is returned in damaged condition, the borrower and/or
agency will be billed for the replacement cost. Videos must not be
loaned out to another individual and must remain in the State of
Oklahoma. Videos returned by mail must be sent via INSURED MAIL
@ $400 per video.
Videos
returned in person must be turned in and processed through Area
Prevention Resource Center staff. Videos turned in to another
area/division shall be construed as missing, and the borrower and/or
agency will be billed accordingly.
